1.Operator didn't put the motor rivet into the tray,in the meantime,the switch riveting workbench is not clean,leads to leave some motor rivets on workbench,when operator was running riveting with taking the switch assy and motor riveting together,one motor rivet mixed in welding process was stuck under the switch terminal; aside from that, because of no critical inspection around the switch, it leads to not check out in subsequent handlings.

1. The operator did not place the motor rivet in the plastic tray. In the meantime, the switch riveting workbench was not cleaned, as a result some motor rivets were left on the workbench. When the operator was performing the riveting, the rivets on the workbench and switch components were picked up. One of the rivets left on the workbench was stuck underneath the switch terminal. This defect was not detected in the following processes because the critical inspection did not include the inspection of the outside of the switch.  
2. Update 5S requirements. All the motor rivets shall be laid in the plastic tray, and it is forbidden to place the motor rivets on workbench in order to prevent the rivets from being mixed up with other parts and/or components. This is one of the key routine inspection items on the 5S checklist.
3. Revise technological document. Prior to the computer inspection, 100% visual inspection shall be performed.
